This plan provides the documentation and the step-by-step procedures needed to implement a QA program that can improve the safety and quality of care in your Pharmacy.

II. Remote Assistance Plan: Consultation and help desk
This plan provides optional services for pharmacies that are subscribing to the basic plan or developing their own QA program.
Telephone, internet, or email access to a pharmacist specialist in medication safety and quality assurance to discuss general QA matters, specific safety/quality concerns, data analysis, systems improvements and other matters. Conversations and communications will be confidential, secure and structured under the pharmacy's QA plan to maximize the afforded legal protections. Up to 4 hours of consultation are provided annually. III. Intermediate Plan: Individualized data review/analysis
This plan provides optional services for pharmacies that are subscribing to the basic plan or developing their own QA program.
Conduct one annual review of prospective QA plan and retrospective analysis of error reports and quality audits. Provide confidential written recommendations on QA plan, necessary modifications based on changing legal requirements, review of systems-improvements in the previous year and strategic plans for safety improvements in future years. IV. Comprehensive Plan: Onsite consultation
This plan provides optional services for pharmacies that are subscribing to the basic plan or developing their own QA program.
Focus is on preventative measures. Conduct staff training in medication safety, systems/risk management review, analysis of quality and adverse event data, executive consultations and development of strategic action plans to improve quality and safety. Provide written summary of visit and 2 follow-up phone calls to reinforce or support changes and action plans. V. Error Management and Recovery Plan
This plan provides optional services for pharmacies that are subscribing to the basic plan or developing their own QA program.
Provide consultation, causal analysis, risk management and systems-improvement recommendations following a significant medication error or significant "close call" error.
